Interdisciplinary team is a collaboration of groups from various sector of healthcare with diverse education and training so as to perform one task with perfection. The group can include social workers, nutritionist, nurses (both practitioners and registered), pharmacists, therapist, clergy and doctors. Team members do work together to coordinate delivery of efficient of services to patients (Flaherty, 2014) In Geriatric Care Team is the same as the Interdisciplinary team the members are concerned with looking at several aspect in patient life. Some of the aspect that the teams ascertain on the patient life making sure that the patient as eaten, dressing the patient, bathing, and the team strive to serve each patient at its best (Arbaje, 2010). Each group of specialized in the team is dedicated individuals who are skilled in their field of profession thus careering for the old becomes an easy task. The paper intends to discuss how the interdisciplinary teams care for Mrs. Randall, an elderly patient in need of Geriatric care.
The multidisciplinary team is known to care for elderly patients and surgery patients; this is because the two groups always need sophisticated health care services (Arbaje, 2010) The team thus is specialized in managing and satisfying the complex need of such individuals. If we take Mrs. Randall case into consideration where flu has been a problem and the facility lacks portable x-ray to cater for Mrs. Randall rare case (Flaherty, 2014). Thought, the condition at the hospital are a little bit off for Mrs. Randall conditions her daughter still argues that the hospital provides the best conditions for her mother, why and what makes her satisfied for with team duty dispute the conditions.
The best care model for Mrs. Randal is the GRACE care Model. This is because Mrs. Randal is under the care of her daughter. Moreover, this type of healthcare is designed to assist low income seniors and is very crucial in decreasing healthcare use. This is something that Mrs. Randal is already yearning for having faced other medical conditions. The best thing about this model is that it provides for home based care. This basically means that, with the help of a physical therapist or a nurse, her daughter can be in a position to take care of her while at home.
The daughter was convinced to let her mother stay in the same nursing home because of the assessment and care that her mother will receive from the home that will eventually lead to her recovering (Flaherty, 2014). The team is known to coordinate in such a manner that the health care of the individual is the top priority of the whole team (Arbaje, 2010). Taking Mrs. Randall case, though the hospital lacks some important facilities that are required to cater for her conditions at her age, through the team members her health can still be prolonged. Her conditions can be improved through the comprehensive care, coordinated plan, unaltered attention and emphasis on palliative care.
The older people are an important part of the society; the individuals are likely to suffer from some complex illnesses that require medication from special practitioners. The illnesses that face older personnel like Mrs. Randall are known to have special treatment and formulation hence the need for the special plan to cater for the groups of individual in the old age care. The special care is because the geriatric care patients are always confused due to their old age and thus the interdisciplinary team is of great essence for their care and maintenance. The comprehensive care offered by the palliative unit of the interdisciplinary care is of great essence for the old age mostly at the end of life. The main aim of geriatric care improves the health of the concerned patient as well as satisfying the emotional benefit of their relatives. This is because this will lower the chances of a patient being involved in nursing home placement for a long period. The models employed by the Geriatric team for time immemorial have proved to be successful despite the minimal setbacks often incurred and that why Mrs. Randall daughter will always prefer palliative care hospital than home-based care.
